Case Manager - Heap



Reports to

Heap Administrator

Job Summary

Under general direction, assist in supporting client and department community referral and resource needs; contact clients that are referred from Heap intake and assess needs based on information gathered; identify appropriate resources to meet needs; connect caller with needed services; provide referral sources and information to staff to assist clients; assist in providing customer service training and support to department; monitor quality of client and Heap staff interaction; assist with reports to funders; collaborate with Quality Assurance team to identify training needs; perform other related duties as assigned.


Completion of a four (4) year degree in social work, or related field and two (2) years’ experience in the provision of social services or equivalent combinations of training and/or work experience. Must have a licensure in social work or be a Licensed Chemical Dependency Counselor II or III or eligible to take licensure exam; State of Ohio Driver’s License and transportation; fingerprint and background checks conducted; Summit County residency required within 12 months of hire.

Pay Rate

$20.98 per hour (Pay Level 10)
