JOB SUMMARY
Pathways Community HUB represents a network of care coordination agencies focused on reaching those at greatest risk and addressing their identified risk factors. The model uses 20 Pathways that address the social determinants of health to improve health outcomes. Under administrative direction, Community Service Specialist (Pathways HUB Referral Coordinator) serves to facilitate communication between referrals sources and contracted care coordination agencies; implements referral intake process; oversees allocation of referrals to community based care coordination agencies; assists with client enrollment; performs intake of referrals including collection of patients demographic, insurance/financial information, etc.; identifies community resources to clients; conducts follow-ups and tracks referrals outcomes; participates in community outreach events; performs various clerical support functions; assist in preparation of reports, presentations, and other related documents and functions; performs other related duties, as assigned

REQUIREMENTS
Completion of post-secondary education with an Associate Degree or equivalent combinations of experience and/or education and a minimum of one (1) year work experience in human services organization. Proficient computer/word processing aptitude (i.e. ability to work in MS Applications such as MS Word, Excel, Outlook) with data entry/retrieval operations. Valid State of Ohio Driver’s License and transportation (travel may be required for observation, consultation and training); drug-test, Criminal background checks and post-employment physical conducted.
PAY RATE
$15.51 per hour (Pay Level 7)
INCENTIVE
$2,000 sign-on incentive ($1,000 at first pay check after hire and $1,000 after six months of consecutive work) and 10% temporary salary increase, per pay period
CLASSIFICATION/SCHEDULE
Regular, Full-Time / Non-Exempt / Monday–Friday / 8:00 a.m.–5:00 p.m.
